Faith Formation Sign Up Page

Registration is now underway for the 2021-22 session. At the moment, it is anticipated that classes will start on September 18th. Registration can be done online for those with access to My Own Church (https://brooklyn.parishsoftfamilysuite.com) or by completing a registration form at the parish office or Download the form below. You may also request the form through email by contacting Paul Cerni at: faithformation@rhcatholic.org

Registration fees (valid until the end of August) are: 

$60 for one child 

$85 for two children 

$110 for three or more children in the same family

Cash, checks, or money orders are accepted as well as credit cards and debit cards.
